Transcatheter closure of patent foramen ovale for migraine prophylaxis: hope or hype?
نویسنده
چکیده
igraine has generally been conceptualized as a primary isorder of the brainstem that mediates sensory input and eural influences on cranial vessels. It is estimated that ,500,000 patients in the U.S. have at least one migraine per eek, with a lifetime prevalence of 18%. Migraine is a risk actor for cryptogenic stroke, particularly in young patients ithout atherosclerosis risk factors. In fact, the prevalence of ubclinical lesions in the cerebellum in patients with miraine with aura, particularly women, is up to 15-fold higher han controls (1). However, the underlying pathophysiology f migraine is not well understood, and new paradigms in nderstanding the underlying etiology and optimizing therpy are being explored.

Effectiveness and Safety of Transcatheter Patent Foramen Ovale Closure for Migraine (EASTFORM) Trial
We evaluated the safety and effectiveness of transcatheter patent foramen ovale (PFO) closure for the treatment of migraine in a Chinese population. This non-randomized clinical trial enrolled 258 consecutive substantial or severe migraineurs with a right-to-left shunt (RLS) (grade II-IV) and grouped subjects according to their election or refusal of PFO closure. Migraine was diagnosed accordin...
متن کامل“Migraine side effect” and stroke recurrence in late follow-up after transcatheter patent foramen ovale closure as secondary prevention of cryptogenic thromboembolism
INTRODUCTION Patent foramen ovale (PFO) is associated with cryptogenic strokes, recurrent transient neurologic deficits, sleep apnea, decompression illness and migraines with aura. AIM We verify cryptogenic embolism recurrence after transcatheter PFO closure in patients younger than 55 years old, and determine the prevalence of migraine with aura before and after PFO closure. MATERIAL AND M...
متن کاملMigraine headache relief after transcatheter closure of patent foramen ovale.
OBJECTIVES The purpose of this study was to determine the effects of transcatheter patent foramen ovale (PFO) closure on migraine frequency in patients with paradoxical cerebral embolism. BACKGROUND The prevalence of migraine headache is higher in cryptogenic stroke patients with PFO than in the general population. Previous studies have suggested that closure of the PFO may reduce migrainous ...
متن کاملIntermediate and Long-Term Results of Transcatheter Closure of Patent Foramen Ovale Using the Amplatzer Patent Foramen Ovale Occluder: One Case of Pulmonary Embolism Irrespective of Patent Foramen Ovale Closure
BACKGROUND AND OBJECTIVES Patent foramen ovale (PFO) has been implicated in the pathogenesis of cryptogenic stroke or transient ischemic attack (TIA) due to paradoxical embolism, and in the pathogenesis of migraine. This paper reports the intermediate and long-term results of transcatheter closure of PFO associated with cerebrovascular accidents (CVAs), TIAs and migraine, using the Amplatzer PF...
متن کاملPrimary transcatheter patent foramen ovale closure is effective in improving migraine in patients with high-risk anatomic and functional characteristics for paradoxical embolism.
OBJECTIVES In the present study, we sought to assess the effectiveness of migraine treatment by means of primary patent foramen ovale (PFO) transcatheter closure in patients with anatomical and functional characteristics predisposing to paradoxical embolism without previous cerebral ischemia. BACKGROUND The exact role for transcatheter closure of PFO in migraine therapy has yet to be elucidat...
